Thank you for attending the Clinical Connections Course.  In order to pass the course, students are required to attend each full-day session and complete the online evaluation for each of those sessions within the specified timeframe of site availability. The purpose of this website is to allow students to complete the session evaluation online.

This site will be available two weeks after lecture.

Note: To pass you must register to this site and complete the Session Evaluation.

How to Complete the Evaluation:

1. To claim credit, click on the Register button in the Activities box.

2. Complete the Site Registration Form, which will provide you access to the Session Evaluation.

3. Create your log in and password for future session evaluations.

4. Complete the Evaluation by clicking on the evaluation link located on the right hand side of your screen under Step 3 Assessments.

5. Upon completion of the Session Evaluation, your Certificate of Evaluation Completion will be generated in PDF format located under Step 4 Certificates.

6. Click on the certificate link to view, print and save your certificate.
